Due to the recent narrowing of networks by many insurance carriers, please be advised that our providers may not be contracted with each and every plan offered by the insurance carriers listed below. Please contact your insurance carrier prior to your visit to confirm your coverage and determine your payment responsibilities.
If you would like for our office to consider contracting with an insurance carrier that is not listed below, please contact our front desk to submit your inquiry. During check-in, if any of your insurance information has recently changed, please inform our front desk so that we can make the necessary updates in our system. Please be aware that with any insurance carrier, there may be deductibles, copayments, and coinsurance for which you could be responsible. We currently accept cash, check, debit, or credit card. Please note that prepaid cosmetic services are non-refundable.

What billing or insurance information will I receive?
You will receive a copy of your bill. After your insurance has paid, we’ll provide you with a monthly statement of your account.
How long will it take to get things settled with the insurance company?
Generally, it takes from 45 to 60 days to obtain payment from an insurance carrier.
Why am I getting bills from laboratories?
Each laboratory who provides professional services will send you a separate bill. This is common if we order bloodwork or take a biopsy. We will do our best to work with your insurance carrier to coordinate your laboratory services with an in-network, contracted provider. Please tell your provider at the time of your visit if your would like for your orders to go to a particular laboratory. Please direct any questions that you have regarding your laboratory bill to the laboratory.
I received a notice that my insurance company has paid on my bill — but I can’t understand how they calculated their payment amount. Do you know?
