If you would like to request that your medical records be sent to another provider, facility, or individual, or that our office complete certain medical paperwork on your behalf (e.g. cancer policy, FMLA, short term disability, etc.), please complete the Medical Records Request Form-To Another Facility and fax it to our Medical Records Fax Line at 904-306-5778.
